1. International passport (valid for not less than 1.5 years)
2. CURRICULUM VITAE (blank is attached)
3. Diploma (notarized copy of your diploma)
4. Diploma supplement with marks
5. Archive transcript (taken from university you've graduated from)
6. Certificate of internship
7. Certificate of medical resident (if you have one)
8. Category
9. Primary specialization
10. All medical trainings
11. Certificate of specialist (if you have one)
12. Copy of employment history book (records here should match your CV)
13. Candidate degree (if you have one)
14. Doctorate degree (if you have one)
15. Prof. degree (if you have one)
16. 20 colour photos 3?4 in white font to take with you to Angola
17. 6 colour photos in white font to take with you to Moscow for preparing working visa
18. Statement of health from narcologist
19. Statement of health from psychoneurologic dispensary
20. Statement of health from tuberculosis dispensary
21. Certificate of vaccination against yellow fever (vaccination is made after approval of flight for work)
22. Medical statement of health (form 082U – blank is attached)
23. Certificate of no criminal record from police
24. Application form (blank is attached)
25. Declaration form (blank is attached)
26. Travel specialist must have an open foreign currency account.

Additional requirements for preparation of the documents.

1. Only a complete set of colour copy documents should be submitted for review.
2. Documents in national language should be translated into Russian and notarized.
3. Do not archive documents.
4. Do not send photos of documents.
5 Do not merge documents into a single image while scanning.
6. Links to some web-sites with applicant's CV and other documents are not acceptable.
7. Send documents only from your own e-mail address.
8. Your full name, specialization and document list should be mentioned obligatory in your e-mail message and its subject field.
